With an annual payment of $255.00 per year, you and your loved ones will now have a benefit of receiving dental treatment at the discounted price.

This plan includes,

1. Two Dental Exams
2. Two Dental Cleanings ( in the absense of Bone loss)
3. Necessary X-rays
4. Discounted Dental Service (20 - 40 % from the UCR schedule)



Here is a step by step procedure to begin your plan.

1. Decide.
2. Go to the payment link (Option 1) on the right.
3. Complete your payment process using the Paypal.
4. Print out your receipt.
5. Call 847-235-6859 to make your first appointment and tell us that you have the JHK Membership Plan.
6. Begin the process to save your teeth!!
